Workers Comp and Third Party program, we can help.

• Doctors Referral for required treatment
• Insurance Company & Claim Number
(claim number may be unavailable if claim is recent)

• All relevant contact details
Payment
• Insurance Company will be invoiced monthly for treatment
Responsibilities
• Client should attend all appointments and take note of cancellation policy. Should client not attend appointment, the insurance company will be informed.
